Anterior Cruciate Replacement The Anterior Cruciate Ligament is most commonly ruptured (torn) in various established sports, notably football, rugby and tennis.

It is also one of the more serious injuries sustained whilst participating in extreme sports such as skiing, kitesurfing, motorcross and mountain biking where the knee is often exposed to more dynamic forces.

The Anterior Cruciate Injury The ACL is a strong ligament, connecting your lower thigh bone on to the lower shin bone. It requires great pressure and force to rupture it. An ACl injury occurs when these bones twist in opposite directions under full body weight, or sudden sidways impact.

Hence the "popping" noise when it goes as you feel your knee dislocate for a split second, which the outer half of the knee actually does!

ACL Initial Treatment Post injury the knee will immediately swell up, this is due to internal bleeding. What is not known at this stage is the degree of damage, not just to the ACL but to the surrounding cartilage. Initially the goals are to reduce the swelling and pain with ice and anti-inflamatories.

As soon as the swelling has eased it is imperative to have an MRI to ascertain the severity of the injury. A scan will enable the Consultant to assess not just the ACl but possible cartilage tears, as there are advantages to some tear types if repair is done early.

ACL Post Injury / Prehab At about 6 weeks the knee will feel good, but the all important ligament is no longer attached. Patients will still typically be able to run in a straight line and cycle. However any rapid turning movement to either side could result in the knee giving way.

Most sportspeople will have a substantial amount of existing muscle to support the knee and subsequent lack of an ACL, however it is most important in advance of an ACL reconstruction to strengthen the knee in a period of Prehab.

Anterior Cruciate Ligament Reconstruction Operation Once the ACL is torn it will not heal by itself, nor is it possible to repair the two ends together. The old ligament ends are removed and a new ligament in constructed using spare tissue from another part of your body.
